It only triggers when direct healing actually restores hit points; overhealing and trivial healing cannot create a large free refill. The maximum pool-based return grows from 1 percent of max mana at 700 to 2 percent at 1250, 3 percent at 1750, and 4 percent at 2000. The amount is also bounded by the actual healing performed.
